Partner Perspectives Podcast – Guest Release and Consent Form

Thanks for being part of Partner Perspectives. This form explains how your interview may be recorded and used, and helps make sure everyone is on the same page.

This Guest Release and Consent Form ("Agreement") is between Partner Perspectives, its host(s), producers, and team (the "Podcast"), and you, the undersigned guest ("Guest"). If the Guest is under 18, this Agreement is completed by a parent or legal guardian ("Guardian").

1. Participation and Recording

By participating, the Guest agrees to take part in an audio and/or video interview or appearance for Partner Perspectives(the "Recording"). The Recording may include the Guest’s name, voice, image, likeness, and anything the Guest chooses to share.

If the Guest would like to remain anonymous, this must be communicated before recording begins, and the Podcast will make reasonable efforts to accommodate that request.

2. How the Recording May Be Used

The Guest gives the Podcast permission to record, edit, and use the Recording, in whole or in part, forever and worldwide, without payment. This includes use in podcast episodes, video, clips, social media, promotion, marketing, websites, streaming platforms, archives, and future formats or platforms.

The Podcast will always own the Recording and all related rights. That ownership cannot be taken back.

The Guest is welcome to share or repost the Recording or clips for personal or professional use, as long as the content is not changed in a way that alters its meaning or suggests the Podcast endorses something it does not.

The Guest understands that the Podcast makes final decisions about editing and how the Recording is used.

3. No Compensation

The Guest understands that participation is voluntary and that no payment or other compensation will be provided.

4. Guest Responsibilities

The Guest agrees that:

  • They are sharing their own views, experiences, and opinions;

  • They have the right to participate and agree to this form;

  • Their participation does not knowingly violate anyone else’s rights.

5. Responsibility for Statements

The Guest understands that the Podcast does not fact-check or independently verify everything shared during recordings. Each guest is responsible for their own statements.

The Podcast, its host(s), and producers are not responsible for any inaccurate, misleading, or defamatory statements made by the Guest or other guests. The Guest agrees not to hold the Podcast responsible for claims arising from the Guest’s own statements.

6. Release of Claims

By signing this form, the Guest agrees not to bring claims against the Podcast related to the lawful use of the Recording, including claims involving privacy, publicity, or defamation.

7. If the Guest Is a Minor

If the Guest is under 18, the Guardian confirms that they are the minor’s parent or legal guardian and have the authority to give permission.

The Guardian agrees to:

  • Allow the minor to participate in the Recording;

  • Grant the permissions described in this form on the minor’s behalf;

  • Accept that the Recording may be used indefinitely without further approval;

  • Release the Podcast from claims related to the minor’s participation.

8. Governing Law

This Agreement is governed by the laws of the state where the Podcast is primarily produced.

9. Entire Agreement

This form represents the full understanding between the parties about the Recording and replaces any prior discussions or agreements.

10. Electronic Signature

This Agreement is completed online. Typing or entering a name below counts as a legal and binding electronic signature, just like signing a paper document.
